After nearly kissing in the on-call room, J.D. and Elliot continue to question their commitments to Kim and Keith, respectively. Elliot has an epiphany, realizing she carried on a meaningless relationship solely due to her desire to be married; consequently she dumps Keith and calls off the wedding. Meanwhile, J.D. realizes his tendency to sabotage himself and yearns to avoid it, even if only for a day. As J.D. and Elliot struggle with these issues, Carla is there to support them, telling them their relationship problems are only human.

Meanwhile, Turk is challenged with choosing his semi-annual candy bar due to his diabetes. He, too, has an epiphany and decides that, rather than having to choose the candy, he can let the candy choose him by making a random selection, ending up with a "Bit O' Honey" bar.

In addition, Dr. Cox, Turk, and Dr. Kelso all pull together to diagnose a lovable patient. After hours of searching through medical books, Cox realizes his initial diagnosis of lyme disease was correct, citing a common adage, Occam's Razor, that the simplest solution tends to be correct; in this case, the patient's tick bite was not discovered because it was under his hair.

Janitor claims to be dating a woman named "Lady." Although J.D. does not believe him at first, it appears Janitor is being honest with J.D.

With Keith angry at Elliot, she seeks ways to gently let him down; she ultimately fails, however, and sleeps with him and makes plans for a date. Realizing she is entering a cycle, Elliot breaks up with Keith again.

J.D. consults Turk on his relationship with Kim, who explains to him that he has been struggling to commit because he does not truly love her; he is only with her because of the baby, though J.D. refuses to believe this. At the end of the day, J.D. realizes he has yet to sabotage himself, yet becomes concerned with his relationship with Kim. Much like his friends, J.D. has an epiphany, citing Occam's Razor once again as he finally realizes Turk was right and he has to stay with Kim, even if he does not love or trust her.
