KBSI (Fox 23) is the Fox television affiliate in Cape Girardeau, Missouri, serving a large area consisting of much of Southeast Missouri, far southern Illinois, the Jackson Purchase of far western Kentucky, northern regions of West Tennessee, and parts of northeast Arkansas. It is owned by the Sinclair Broadcast Group, and is the sister station to WDKA in Paducah, Kentucky. KBSI has been serving the area since 1983. Its transmitter is located north of Cape Girardeau.

Sinclair and Fox recently finalized a six-year affiliation contract extension for Sinclair's 19 Fox affiliates, including KBSI. KBSI's affiliation contract now expires in March 2012.
