This page is intended for reports of usernames that are blatant and serious violations of the username policy requiring an immediate block. Reports will be assessed in accordance with the username policy, the UAA instructions, and the following bullet points. Please ensure you are familiar with the assessment criteria before making a report.
- Except in the instance of an egregious name violation, please do not report accounts with no edits or those who have not edited in the preceding 2 weeks.
- Real names are permitted, except when the editor implies they are someone other than themselves, such as impersonating a notable living person.
- Promotional names require evidence to be blocked. Do not report a username merely because it "appears" promotional.
- For libellous usernames or usernames that contain another editor's nonpublic personal information, please contact an oversighter and do not report it here.
- Discuss less-serious violations with the user on their talk page so that they can rename or abandon their account in good faith. Templates such as {{Uw-username}} or {{Uw-coi-username}} may be used. If, after discussion with a user, the problem still seems unresolved, a username request for comment may be in order.
- Do not leave a username warning on a user's talk page and also immediately report them here. Do one or the other, and not both simultaneously.
- Patrollers are asked to remove reports that have been declined, are bot-reported false positives or are otherwise non-blatant or stale reports.
Note: Administrators should also kindly consider usernames listed at Filter 149, Filter 188, Filter 354, WP:UAA/HP and CAT:UAA.

User-reported
- TastyCawks (talk · contribs · deleted · filter log · SUL · Google) • (block · soft · promo · cause · bot · hard · spam · vandal) — Violation of username policy because it's an offensive username; "Cawks" is pretty obvious — "cocks" in case you didn't get it. Surprised this one wasn't caught earlier; user has been editing sporadically for 10 months.. Ten Pound Hammer, his otters and a clue-bat • 02:28, 31 December 2010 (UTC)
- Note: User is in the category: Wikipedian usernames editors have expressed concern over. HBC AIV helperbot7 (talk) 04:18, 31 December 2010 (UTC)
- Being discussed with the user. When someone has been editing that long, even this minimally, with no other issues, we should discuss this first. And are you so sure about this? It wouldn't hurt to ask first. Daniel Case (talk) 04:21, 31 December 2010 (UTC)
- Note: After going through his edit history, I agree with Daniel. Maybe he just likes caulk. Or cocking a firearm. Or eating roosters. :-) KrakatoaKatie 04:33, 31 December 2010 (UTC)
- Faded Glory USA (talk · contribs · deleted · filter log · SUL · Google) • (block · soft · promo · cause · bot · hard · spam · vandal) — Violation of username policy because it's a promotional username; Faded Glory USA is the name of a US Clothing Manufacturer - this is a promotional nick. No edits, better to stop now. . BarkingFish 22:38, 30 December 2010 (UTC)
- Note: User is in the category: Wikipedian usernames editors have expressed concern over. HBC AIV helperbot5 (talk) 23:01, 30 December 2010 (UTC)
- Being discussed with the user. They requested adoption, so they appear to be a good faith editor. I'll try discussing with them first. --Ks1stm (talk) 23:02, 30 December 2010 (UTC)
- Note: I would have also advised the original reporter to wait ... I agree it is a strong coincidence, but what if they start editing something that has nothing to do with clothing or Wal-Mart? If they are promotionally intended and never edit, no harm no foul. If they are a good-faith editor who thought the name of their favorite clothing line would be OK, we can persuade them to change their name. Daniel Case (talk) 04:24, 31 December 2010 (UTC)
- Playbill (talk · contribs · deleted · filter log · SUL · Google) • (block · soft · promo · cause · bot · hard · spam · vandal) Same name as a known magazine and edits magazine-related articles. See Playbill. Note: I post here for external assessment and second opinion. My instinct on this is to block as s/he gives the impression, by using the name, to be representing the magazine.
