Phi Delta Chi is a professional fraternity founded in 1883 at the University of Michigan. In the following list of chapters, active chapters are indicated in bold and inactive chapters are in italics.

  4. Chapter formed at Philadelphia College of Pharmacy, that became the University of the Sciences in 1988. In 2002, the university merged into Saint Joseph's University.
  5. Chapter formed at the California College of Pharmacy that became the University of California, San Francisco in 1934.
  6. Chapter formed at the Maryland College of Pharmacy, now part of the University of Maryland
  7. Chapter started in Galveston but followed the pharmacy school to Austin in 1927.
  8. ^ Chapter formed at the University of Nebraska Lincoln but moved to the University of Nebraska Omaha with the Pharmacy School in 1976.
  9. Chapter was formed at the University of Oklahoma in Norman, OK. However, it moved to Oklahoma City with the Pharmacy School in 1976.
  10. ^ Chapter formed at the University of Colorado Boulder but moved with the Pharmacy School to the University of Colorado School of Medicine in Aurora in 1992.
  11. Chapter formed at Alabama Polytechnic Institute that became Auburn University in 1959.
  12. ^ This chapter was formed at the Highland Park College of Pharmacy which merged with Des Moines College (DMC). The Des Moines School of Pharmacy separated from Des Moines College in 1927 and DMC closed in 1929. The Pharmacy College closed in 1939. At that time, the former campus and faculty became part of Drake University, necessitating the chapter's move to Drake.
  13. ^ The chapter formed at the Louisville College of Pharmacy that merged with the University of Kentucky in 1947. In 1957, the University of Kentucky College of Pharmacy moved from Louisville to Lexington, Kentucky.
  14. Chapter formed at Medical College of Virginia that merged to form Virginia Commonwealth University in 1968
  15. Chapter formed at the University of Idaho-Southern Branch that became Idaho State University in 1962.
  16. Chapter formed at the College of City of Detroit that became Wayne University in 1934 and Wayne State University in 1957.
  17. Chapter formed at the South Carolina College of Pharmacy that became the Medical University of South Carolina.
  18. Chapter formed in New Haven but moved to Storrs with the Pharmacy School in 1951.

  20. Chapter formed at Howard College that became Samford University in 1965.
  21. Chapter formed at Northeast Louisiana University that became the University of Louisiana Monroe in 1999.
  22. Chapter was established at the University of Kansas City that became the University of Missouri–Kansas City in 1960.
  23. Chapter formed at the Brooklyn College of Pharmacy, LIU that became the A&M Schwartz College of Pharmacy, LIU Arnold and Marie Schwartz College of Pharmacy in 1976.
  24. Chapter formed at the Southeastern University of Health Sciences that changed its name to Nova Southeastern University in 1994.
  25. Chapter formed at the Nevada College of Pharmacy in Las Vegas which changed its name to the University of Southern Nevada in 2004. It moved to Henderson, Nevada in 2005 and changed its name to Roseman University of Health Sciences in 2011.
  26. Chapter is affiliated with the Auburn University program held at the University of South Alabama.
